3. I am informed that the session can be conducted through text, audio, or video communication.
4. I agree that my medical records on telehealth can be kept for further evaluation, analysis and documentation, and in all of these, my information will be kept private.
5. I am informed that to provide me with the best possible virtual experience, some of my personal information (i.e. IP address) may be processed and stored outside my jurisdiction.
6. Please be advised that Your Doctors Online telemedicine services are not suitable for medical emergencies or conditions requiring urgent medical attention. If you require immediate medical attention, please contact your local emergency services. Our platform offers consultations for non-urgent medical issues and general health inquiries. It is important to provide accurate and complete information during your consultation to ensure the best possible care and advice from our healthcare providers. However, please note that remote consultations have limitations, including the inability to conduct physical examinations and prescribe certain medications. By using our services, you acknowledge these limitations. For urgent concerns, please seek in-person care or contact your primary care provider.

Reliance on Your Doctors Online Medical Assessment

1. While based on your information, it's just a guide and not a substitute for an in-person doctor's visit.
2. Doctors need physical exams and tests for accurate diagnosis, which online services lack.
3. When seeing a nurse practitioners or physician assistants, they are qualified healthcare providers supervised by licensed doctors.

Consultative Service

1. No guaranteed results or healthcare outcomes.
2. Provider may deem the service unsuitable or unable to provide the required services.
3. Provider may discontinue consultation at any time.
4. Diagnosis is limited and provisional, not a replacement for a full medical evaluation.

Children

1. You must be at least 16 years old to access the service.
2. Children under 16 cannot use the service unless added as a dependent by a parent.

Credit Transactions

1. Only credit cards are accepted as payment methods.
2. Subscriptions are billed automatically unless cancelled at least one day before the renewal date.
3. Our services, including the Dollar-a-Day Doctor and Sick Note Plan and the Yearly Family Care Plan, are annual subscriptions. Upon activation, payment for the entire year will be charged to your credit card.
4. Our services include plans that are billed monthly based on an annual term commitment. By enrolling in an annual plan, you acknowledge that you are committing to a full twelve-month service period; consequently, any cancellation of a monthly-billed plan prior to the completion of said term will be subject to a termination fee equal to 50% of the value of the annual contract.
5. This annual fee is nonrefundable once any part of the service has been used.
6. By signing this consent form and subscribing to any of these services, you agree to be charged for the full year upfront.
7. Access to services is contingent upon successful payment.

   2. Non-Eligibility: This service is not suitable for individuals experiencing:
      1. Severe eating disorders requiring intensive in-patient care, such as anorexia nervosa or bulimia.
      2. Conditions that require direct medical intervention or complex dietary management, such as severe food allergies, uncontrolled diabetes, or significant kidney disease.
      3. Individuals undergoing any legal mandates for treatment, such as court-ordered nutritional therapy or cases needing forensic assessments.
      4. Individuals with severe cognitive impairment or intellectual disabilities.
   3. Continuity of Care: This service is not intended to replace ongoing medical or dietary care you may be receiving.
3. Confidentiality and Data Use

Subscription Cancellation and Refund

1. All Single Visit, Monthly, and Quarterly plans are non-refundable. By proceeding with payment, you acknowledge and agree that no refunds, full or partial, will be issued once the service has been purchased, regardless of usage or cancellation.
2. We offer monthly, quarterly, and annual subscription plans.
3. Our Dollar-a-Day Doctor and Sick Note Plan and Yearly Family Care Plan are annual subscriptions. Upon activation, the full year's payment is charged upfront, and by signing this consent form and subscribing, you agree to this charge.
4. Our subscriptions are non-refundable once any part of the service has been used, as resources are allocated immediately upon activation. This includes both initial payments and automatic renewals.
5. For our subscription plans, cancellations must be made before the renewal date to avoid being charged for the next billing period. You will retain access to services until the end of the current paid period.
6. Refunds are generally not provided for any subscription type. However, in exceptional cases, a refund request may be considered at our discretion.
7. To request a refund, please contact support@yourdoctors.online with the reason for your request. If approved, the refund will be issued to your original payment method within 5–7 business days.
